For a guy who constantly looks nervous it pretty much gives it away, Greyson is living with a cronical depression. It doesn't mean he hides away in the corner of his house, quite in reverse, he's always the one with the biggest smile. Therefore people often can't tell his depression is a real thing until the mental breakdowns happen "out of nowhere" as it is described.
In his head he hears voices. Not like a regular person with an inner voice and a thinking voice. These voices spoke to him already from a very young age. Animals and things with a face who was not supposed to be talking, spoke to him, told him and taught him about the life his parents miserably failed to teach him. His father was a drunk, and his mother committed suicide in front of him because nobody but Greyson understood her. The two of them were close. But to prevent ending up in a mental hospital The voices she heard were the same Greyson heard, and he promised to never tell. But he did. After ending up in fostercare in the home of a lovely family they discovered his secret, and they enrolled him in some sort of treatment with a therapy session every week. He learned about God, and how his existence might help him in a time like this, when his great depression was at its worst due to his mother's death.
